COVID-19 Vaccine Requirement Ending - In accordance with state and federal guidelines, CU Boulder’s COVID-19 vaccine requirement for students, faculty and staff will end effective May 15, 2023.
Vaccinations and boosters continue to be strongly recommended as a safe, effective tool for minimizing the effects of COVID-19, including serious illness and death.
Vaccinations will continue to be required for students and staff working in CU Boulder health care facilities in alignment with federal/state guidance.

Please review your Spring 2025 new student immunization requirements below.
All requirements must be completed by Jan. 14, 2025.
*Failure to complete these requirements by the deadline will result in a registration hold.
Proof of MMR vaccination
Colorado state law requires proof of two (2) valid Measles, Mumps and Rubella (MMR) vaccines OR proof of immunity (titers) for all incoming students.
Proof of MenACWY vaccination
CU Boulder requires proof of one (1) valid Meningococcal Disease (MenACWY) vaccine administered within the last five years for all students under the age of 23 living in university-operated housing.
Tuberculosis Screening Questionnaire
CU Boulder requires all incoming students, regardless of vaccination status, to complete a Tuberculosis Screening Questionnaire and all follow-up tasks.
Enter vaccinations
You must enter the types and dates of the vaccines you have received based on your vaccine record.

Click on 1. Enter Vaccinations from the menu.

Use the dropdown menus to select your vaccine type and enter the date(s) you received each vaccine OR use the Verivax tool to request your immunization records from your out-of-state registry.
Upload proof
You must upload a copy of your vaccination record to verify that you have received all required vaccines.

Click on 2. Upload Proof from the menu.

Review documentation requirements.

Upload your official vaccination record.
*If you have more than one file to share, you will need to upload them separately.
Please note: Students will not get a confirmation that proof of vaccination or vaccine exemption was successfully submitted. Overall status is available on the portal and will reflect Verified once all requirements have been completed and verified. The process of verifying records may take 5 business days.
- Select 3. Complete Forms from the menu.
- Select the option for the Tuberculosis Screening Questionnaire.
- If you are determined to be high-risk, you will need to upload the results of a TB Test administered within the last 6 months. You will receive a secure message with information about obtaining TB testing.
- Your TB test results will be reviewed, and you will be notified if follow-up is required.
This form must be signed by a physician, advanced practice nurse or physician assistant, verifying a medical reason for the vaccine exemption.
This form must be signed by a healthcare provider licensed in the state of Colorado.
Complete this online learning module and provide a copy of your certificate of completion (provided at the end of the module).
To submit your exemption documentation, complete the following steps.
Log into your with your IdentiKey and navigate to Immunization Requirements.
Select 2. Upload Proof from the menu.
Upload your chosen exemption form.
Please note:
If you have submitted a vaccination exemption form for MMR, please do not enter any MMR vaccine information under the 1. Enter Vaccinations menu.
You are still required to complete the Tuberculosis Screening Questionnaire.

If you would like to submit an exemption form for the MMR vaccination requirement, please choose one of the following documents to complete.
If you would like to submit an exemption form for the MenACWY vaccination requirement, log into your with your Identikey and complete the following steps under Immunization Requirements.
Select 3. Complete Forms from the menu.
Select the option for Meningococcal Disease Info and Vaccine Waiver under Optional Exemption Forms.
Complete the online form and click submit.

If you have submitted a vaccination exemption form for MenACWY, please do not enter any MenACWY vaccine information under the 1. Enter Vaccinations menu.
You are still required to complete the Tuberculosis Screening Questionnaire.
